Record Modification Request Form
This form is to be used to modify an existing record due to errors in a description or other wording, the need to add additional attachments, etc. or other changes needed to be made to an existing record. Once the Record Modification Request Form is submitted, TPI staff will make the necessary modifications to the records for you and contact you about the modifications if needed. Do not use this form if there is a new version, model, etc. or if there are significant updates to equipment or a file format. These types of changes will require a new record to be created and that information is to be recorded using either the Equipment Form or File Format Form listed above.
