New & Modified Records Section

Use the forms in this section to add a new record for your equipment or file formats which your equipment consumes There are three forms available as outlined further below.

  • Equipment Form - Use this form to add a new record for your equipment but only after you have verified that the File Formats consumed by your Equipment are listed in Table 3 above. If they are not, you will need to add the File Formats first using the File Formats Form.

  • File Format Form - Use this form to add a new record for the file formats.

  • Record Modification Request Form - Use this form to modify an existing record due to errors in the existing record or if additional attachments, etc. are needed to an existing record.

Equipment Form

Use this form to add a new record for your equipment which will be added to Table 1 above. To view the updated Table 1 you may need to refresh the page after you have submitted the form.

File Format Form

Use this form to add a new record for the file formats your equipment can consume including native formats that your company writes. Each entry will add a new record to Table 2 and Table 3 above. To view the updated Table 2 and Table 3 you may need to refresh the page after you have submitted the form.
